Again, please include as much as you can think of, what they are like with other animals, relationship to animals in the household, relationship to people, what makes them happy, how relaxed are they, are they sensitive to anything - noise, touch, travel, what activities do they like, what makes them anxious, what they are like at the vet, how do they display any of this behaviour? Any bite or scratch history, what level and frequency ie, scratch the skin, puncture the skin, single bite/scratch or multiple.
To check your cat’s respiration rate, count his chest movements (in and out is one breath) for 20 seconds and multiply by 3 to get the total number of breaths per minute. Practice at home, when you and your cat are both very relaxed or your cat is asleep.
For how long has the cat’s behaviour and/or health been a concern? Did the issue/s arise suddenly or have they been progressive? Any incidents or environmental changes you can think of, no matter how small, new animal /neighbour /groomer etc? Is the behavour directed towards any particular family member, strangers, children or other pet in the house? When does it happen, during petting, handling, walking past, carrying, at vets etc? How often are the behaviours occurring, daily, weekly, monthly, randomly, other? What circumstances, times, do you think may predict the behaviour occurring?
What are your training goals, short, mid and long term?
How much time to you have to commit to a behaviour modification program?

If behaviour problems are not resolved to your satisfaction OR should you not have the ability to participate in training what other considerations have you made?

I assure you I'm not interested your personal life rather, because behaviour does not happen in isolation, I'm looking for any/all experiences that are influencing your cat/s behaviour.

Also note type of litter, depth of litter in box, how many litter boxes per cat.
How often do you scoop the litter box( multiple times a day, daily, weekly etc)? How often do you completely empty and replace litter (weekly, monthly)?
Please describe urine, smell, colour, where sprayed, puddles, feces, firm, soft hard, in or out of the box.
If your cat is urinating or defecating outside of the box please describe where they are doing it and on what other materials they are doing it on. Is it hidden in closets or in main areas of the house, in the laundry etc? Are they targeting other pets bedding, toys etc or human or anything you can pinpoint?
Does you cat have a scratching post? how many? Please describe them. Does the cat use it? Does it move or is it sturdy?Does the cat scratch other surfaces? Furniture, carpet, beds, wallpaper, windows, fabric etc please describe.
Are your cat's nails trimmed? If so, how often? Who trims them?
Was your dog allowed to meet and greet dogs on lead as a puppy? Has he had any incidents with other dogs while he’s been on the lead or tied up? What is she like off lead? Any particular breeds /colour /coat type that he reacts to? Does she go to a groomer? How does he cope? Have you ever stayed to watch? Do you live by an AirBnB? Etc etc etc
